How To Become A New Practice Member
Step 1: Fill out the paperwork
Please click the button below to print out the following new patient forms. Fill out the forms and bring them to your first visit. If you are unable to print out the forms, please arrive 15 minutes before your appointment time so you can fill them out in the office.
Step 2: Watch the Orientation video
Watching the Orientation videos is mandatory. If you choose not to watch the video, please choose one of the other great chiropractors in your area. Included in the new patient forms is a brief quiz for you to fill out after you watch the video just to make sure you catch all the key points. Don’t worry, it’s super easy if you watched the video! Please be aware that you will not be accepted as a practice member until you watch the Orientation video and complete the quiz.
Step 3: You arrive for your appointment… what’s next?
When you arrive at the office at your appointment time, you have already watched the Orientation video and most likely have printed and filled out the new patient forms. If you have not yet fill out the forms, you will be given a chance to do so. The doctor will review your paperwork and perform an evaluation. This usually includes evaluating your posture, feeling your spine and surrounding tissue, and analyzing for subluxations. If the doctor deems it appropriate and safe, you will receive an adjustment.
Step 4: You’re All Set!
The power has been turned on! Go out and enjoy your day! We look forward to seeing you in 7-10 days!
